Irvine Health Foundation Prize Submission
To download the Nomination Form to your computer, in Internet Explorer right-click on the hyperlink and choose "Save Target As..."; in Apple Safari right-click on the hyperlink and choose "Download Linked File"; in Firefox right-click on the hyperlink and choose "Save Link As...".
Nominations must be submitted no later than midnight on Saturday, July 31, 2010. Irvine Health Foundation will only accept nominations via the website. Upon receipt, Irvine Health Foundation will e-mail a confirmation of nomination receipt to the nominee following an initial review of the nomination form for completeness.
Irvine Health Foundation will notify all nominees in writing via e-mail and USPS regarding award status in mid November 2010. The Prize recipient will be honored by the Irvine Health Foundation at an event on January 26, 2011.
